Let the patient, elevating the head, and applying cold pack are appropriate orders. Insert nasogastric tube.  Rhinorrhoea may indicate a dural tear with cerebrospinal fluid (CSF) leakage, and insertion of a nasogastric tube will increase the risk for infections such as meningitis.

The appropriate orders which are required are applying the cold pack. You have to insert a nasogastric tube. Cerebrospinal fluid leakage may be seen to appear where rhinorrhea indicates a dural tear.
If nasogastric which is being inserted is increased can bring out a risk of infection. For example, meningitis.
