Actually there are a lot of failures that media were not able to deliver the correct information to the public.
But here are only 5 of it.
- Media deliver only a one side of the story
- Media presents positions.
-  Media attacks the source of the information instead of validating it.
- Media fails to research more proofs about a complex issue. 
- Media falsely present information.
