The inflammatory reaction is a non specific immune response against the entry of pathogen or any damage to tissues caused by physical or chemical factors. It includes the arrival of phagocytes at the site of infection to kill the entered pathogen. The pus includes the dead pathogens, dead tissue cells and some cells of the immune system. Hence, pus was formed on Jane's elbow as a result of the inflammatory response against tissue damage.  

Some of the cutaneous receptors present in the dermis of skin serve as pain receptors. The pain experienced by Jane is actually sensed by pain receptors present in the dermis of the damaged region of the elbow.

This is because Jane's elbow was scratched. The human skin as a normal flora mostly consisting of <em>Staphylococcus aureus</em> and <em>Streptococcus pyogenes. </em>Any breaks in the skin will let the flora invade the inside of the body and cause a local inflammatory reaction and liquefactive necrosis (formation of pus). Sarah's knee was fine because the knee is just bruised but the skin remained intact and the normal flora cannot invade an intact skin.
