Ms. S, a 78-year-old black female, is brought to the emergency room by her son who reports that she has been vomiting violently
lawyer [7]

Answer:

1. Severe dehydration

2. Investigations required will include serum urea, electrolytes and creatinine (U/E/Cr), full blood count and differentials, urinalysis, urine microscopy, culture and sensitivity, renal ultrasound, random blood glucose.

3. Deranged U/E/Cr that is urea, creatinine and sodium are elevated which indicate kidney injury. There is proteinuria, haematuria, positive nitrate and leucouria points towards UTI. The WBC is high which indicate infection.

5. She is likely to have prerenal AKI

6. The concern is that she is not regular on her antihypertensive medication, and hypertension can cause renal impairment ( hypertensive nephropathy). There is need for dialysis if there is markedly elevated E/U/Cr or renal impairment was very severe.

Explanation:

Acute kidney injury is when there is sudden and often reversible loss of renal function which develops over days or weeks and is usually accompanied by a reduction in urine output.

There are many causes of AKI, which are classified as prerenal, renal and post renal. In prerenal AKI, the kidneys gets damaged as a result of hypoperfusion leading to acute tubular necrsosis. Common examples are severe dehydration( like in this patient from vomiting), cardiac failure, sepsis, blood loss etc. Renal AKI is usually cause by direct insult to the kidneys which can result from drugs, toxins, inflammatory conditions etc. Post renal AKI occurs when there is obstruction to urine flow at any point from the tubule to the urethra e.g in BPH, stones, strictures etc.

In Prerenal AKI, it leads to acute tubular necrosis. Histologically, the kidney show inflammatory changes, focal breaks in the tubular basement membrane and interstitial oedema.

Chronic kidney disease(CKD) refers to an irreversible deterioration in renal function which develops over a period of years.initially, it is manifestation only as biochemical abnormality but eventually, loss of the excretory, metabolic and endocrine functions of the kidneys leads to clinical symptoms and signs of renal failure.

3. How does having multiple methods of reproduction support the longevity of sponges in the fossil record?
Tomtit [17]

Multiple methods of reproduction support the longevity of sponges in the fossil record is described below.

Explanation:

Sponges are able to reproduce both sexually using gametes and asexually by budding. Even though sponges are hermaphroditic, individuals will only make one type of gamete at a time. There are two forms of asexual reproduction that sponges can go through: external budding and internal budding.

Sponges reproduce sexually, too. Sponges do not have separate sexes-a single sponge forms eggs at one time of the year and sperm at a different time. At any one time of the year, some sponges are producing eggs and others are producing sperm.

The volume of water passing through a sponge can be enormous, up to 20,000 times its volume in a single 24 hour period.

Sponges reproduce by both asexual and sexual means. Most poriferans that reproduce by sexual means are hermaphroditic and produce eggs and sperm at different times. Sperm are frequently "broadcast" into the water column. ... Some sponges release their larvae, where others retain them for some time.

Threonine is a standard amino acid with a polar side chain.Identify the chiral centers in threonine. a. the carboxyl carbon b. t
luda_lava [24]

Answer:

Threonine has two chiral centers.

The correct options are c. carbon b and d. carbon c

Explanation:

Chiral centers are those that have four different groups of atoms bonded to it. Carbon a has three atoms of hydrogen attached to it so it can't be a chiral carbon. It isn't the carboxyl carbon either since it is double bonded to oxygen. And the amino nitrogen does not qualify anyway because it is bonded to three different groups of atoms, not four.

Explain have a fats sugars and or starch contained in seeds or milk are useful for the plant sprouting from the seed or the baby
Kipish [7]
There is a lot of calories in fats that can be a good help and can be used for growth, fats are high energy molecules that help in growing organism. Fats produces carbon dioxide, water, and ATP when metabolism.  The sugars and or starch is useful in the same method, they provide calories necessary for a seed or baby animal to grow. The sugars used in glycolysis and then the products of glycolysis used in  the citric acid cycle, and the starch can be cleaved into more manageable sugars.
